Hospital trust says it has learnt lessons after death from side-effects of the pillpublished at 17:53

BBC News OnlineThe Burton Hospitals NHS Foundation Trust has expressed its "deepest sympathy to the family of Fallan Kurek".
The trust said it had investigated the circumstances of her death and had "established a robust action plan" to increase awareness at all its hospitals of the risk factors associated with chest pain.

Indefinite hospital order for man who killed his partner in Nechellspublished at 17:25
A man who killed his partner has been given an indefinite hospital order at Birmingham Crown Court today.
Ozekell Empire, 31, was deemed unfit to stand trial following the death of Andrea Carr, whose body was found at their home on Nechells Park Road in December 2014.
A post-mortem test initially proved inconclusive, but West Midlands Police today said "further tests concluded she died of asphyxiation".

The family of Andrea Carr, said: "Andrea is dearly missed and we hope that nobody else has to endure loss in these circumstances. We are happy with the decision of the court but nothing will replace Angie in our lives."

BBC Midlands TodayA Black Country head teacher has pledged to make up the teaching time lost during a bomb scare that affected his and three other schools in the region earlier.
Pupils at Holly Lodge, Oldbury Academy and Bristnall Hall, in Sandwell, were evacuated and lessons at Leasowes High School in Halesowen were halted.
Vince Green, principal of Bristnall Hall, said the school was "disappointed" to have lost the time today. West Midlands Police gave all four schools the all clear by 13:30.

Side effects of the pill 'not effectively treated' caused teaching assistant's deathpublished at 16:31
A teaching assistant collapsed and died from a blood clot caused by taking the contraceptive pill after being sent home from a minor injuries unit with painkillers, an inquest has heard.
In a narrative conclusion, South Staffordshire Coroner Andrew Haigh ruled that Fallan Kurek, 21, died from a "massive" pulmonary embolism after the side-effects of the pill were not effectively treated; the nurse diagnosed her pain as being muscular.
The hearing was told that Ms Kurek, from Tamworth, Staffordshire, was assessed by a nurse at the town's Sir Robert Peel community hospital on 8 May last year after complaining of chest pains and feeling breathless.

Leasowes High bomb scare dismissed as a 'hoax' by councillorpublished at 16:17
Dudley council has spoken to us about the bomb scare earlier at Leasowes High School.
Councillor Ian Cooper, cabinet member for children’s services, said: “Leasowes High school in Halesowen was evacuated this morning after receiving a call which turned out to be a bomb hoax.
“We are working with West Midlands Police on this incident, but there is no suggestion of any other threat to other schools in Dudley borough."

Fallan Kurek, 21, died in hospital in May last year after being found collapsed at her home in Tamworth.
Her father Brian, speaking to the media shortly after his daughter's death, said she had been looking forward to the rest of her life and described her as "confident, cheeky and bubbly".

Two men accused of funding terrorism by sending money to their brother in Syria have appeared in court.
Mohammed Iqbal Hussain, 25, and Mohamed Suyaubur Rohaman, 32, from Walsall in the West Midlands, are alleged to have transferred £10,000.
Mr Hussain also faces three other terror funding charges. Their lawyer indicated they would plead not guilty.

BBC Local LiveBristnall Hall Academy principal Vince Green said it would be "business as usual on Wednesday" after police gave the school the all clear following a bomb scare call earlier.
On today's decision to evacuate pupils, he said: “I spoke to the vice principal and staff told pupils very calmly that we were evacuating the school and they were going home for the rest of the day.
"Children who could not go home were taken to nearby Moat Farm School until they could be collected. Parents were notified by text and social media about what was happening."
He said he was angry and disappointed at what had happened.
