Ed Shervington crashed over for a crucial second-half try as Worcester maintained their 100% start with victory over old rivals Rotherham.
Miles Benjamin' ninth-minute try - his fourth of the campaign - gave Warriors a 10-0 lead, Andy Goode converting then adding the first of six penalties.
Rotherham then hit back with two tries to go in only 16-15 down at the break.
But, despite the Titans taking the lead, Warriors hit back for Shervington to earn a third straight win.
As the only unbeaten team in the Championship, Worcester are already clear at the top, but only by two points - as they have only earned one four-try victory, against Birmingham and Solihull on the first day of the season.
And it could easily have gone the other way, the visiting pack earning first-half dominance from which they benefited when Dan Baines and Rob O'Donnell crashed over for close-range tries.
Warriors were then stunned when a try from Ryan Burrows saw the Titans take a slender lead with less than 20 minutes left on the clock.
But Worcester hit back, Shervington's try, improved by Joe Carlisle, who also added another penalty, seeing them home.
Warriors coach Richard Hill told BBC Hereford and Worcester: "You have got to kill these teams in the first 20 minutes, particularly the first half, so I was disappointed.
"We had a great chance at the end of the first half where we made a superb line break and the final pass did not go to hand.
"That would have broken the back of Rotherham and given us a huge lift. I don't think they would have come back from that."
