Three officers from West Mercia Police who led the operation in which 39-year-old Marcus Meade was shot dead in his own home are being investigated for potential misconduct.
Meade was killed on Christmas Eve 2024 after police entered his flat following a five-hour stand-off. At the time he was in possession of a knife and suffering a mental health episode.
The Independent Office for Police Conduct says it is probing the management of the operation in which Meade was killed.
Family spokesperson, Bishop Dr. Desmond Jaddoo MBE, tells the BBC that they are pleased their questions are being taken seriously.
West Mercia Police say the officers, who led the firearms operation, are under investigation by the IOPC for potential misconduct in connection with the planning and management of the operation.
Assistant Chief Constable Grant Wills said: "Our thoughts and condolences remain with the family of Marcus Meade and all those who have been affected by Marcus' tragic death."
All other armed officers involved in the operation continue to be treated as witnesses.
A suspension review has taken place and no officers have been suspended.
