1) In natural, spoken English, do people always ask full questions ?

No, you are likely to hear shortened forms of questions. For example:
ไม่จำเป็น และอาจจะได้ยินคำถามสั้น ๆ มากกว่า เช่น:

• Would you like regular or large?

Is short for: ซึ่งเป็นรูปย่อของ:

• Would you like a regular or large wrap?

Often, you'll just hear a word or two with rising intonation rather than a full question form. This is very natural in spoken English. For example:

ตามปกติแล้ว เวลาใช้ภาษาอังกฤษแบบพูด เราจะได้ยินคำถามเพียงหนึ่งหรือสองคำที่เน้นเสียงขึ้นสูงตอนท้าย แทนที่จะเป็นประโยคเต็ม เช่น:
