The search for sport's Unsung Hero is taking place across the United Kingdom. It will see winners initially named in the following BBC areas:

Northern Ireland, Scotland, Wales, East, East Midlands, London, North East & Cumbria, North West, South, South East, South West, West, West Midlands, Yorkshire, East Yorkshire & Lincolnshire.

These regional Unsung Heroes will then be invited to the BBC Sports Personality of the Year programme on Sunday 10th December 2006 where an overall winner will be announced.

HOW TO MAKE A NOMINATION

To enter, you will need to complete all sections of the nomination form (note: users reading this article via our mobile service will need to view this page on a pc to enter online).

Send the completed form before the closing date of Sunday 5 November to:

Alternatively, you can complete your application online - remembering that all sections need to be filled in. Any nomination that doesn't include details of the nominee cannot be accepted.

Your nominee must be informed and have agreed to being entered.

Regional winners will be announced on or before Monday 4 December 2006

Nominees may be asked to appear on BBC TV and/or radio.

The judges' decision is final and no correspondence will be entered into regarding their decision.
