Replacement Shane Drahm's late drop-goal capped a fine Worcester comeback and gave the Warriors their first Premiership win of the season.
Saracens led 17-3 at half-time thanks to converted tries from Thomas Castaignede and Glen Jackson.
But Sarries' line-out fell apart after the break and Chris Fortey and then Gavin Quinnell went over for Worcester.
And the visitors' sustained pressure was rewarded when Drahm landed a drop-goal in the 77th minute.
The first league win came in the same week that Worcester sacked head coach Anthony Eddy.
Saracens director of rugby Alan Gaffney:
"We can take a lot of positives from the first 40 minutes but we can't take one from the second half. "There were a lot of dumb actions on the pitch, and until we become a smart side we'll struggle against any team.
"We've got to examine ourselves. We threw that game away, and it will stay with me for the rest of the year."
Worcester rugby director John Brain:
"Saracens are a side in form, but our second-half performance was probably the best in the five years I've been at Worcester. "That win will give us a lot of confidence and now we've got to look forward to taking that firepower into the next three non-Premiership games and build from there.
"If we continue to perform like that we'll be fine and someone else will be in trouble (in the table)."
