Chris Burke scored a 75th-minute winner for Cardiff as they claimed their first win of the season at Pride Park.
Burke set up the opener for Michael Chopra after 15 minutes with a telling run and the striker slotted home with a cool finish from 12 yards.
But 10 minutes later Tomasz Cywka levelled while the visitors' Kevin McNaughton was stretchered off in a neck brace as half-time neared.
Peter Whittingham, Jay Bothroyd and Chopra missed before Burke struck.
That gave Dave Jones' side only their second win in 32 years on Derby soil as well as a first win of the league campaign having drawn 1-1 against Sheffield United - the only Championship draw on the opening weekend.
However, the Rams followed their 2-1 win at Leeds with with a defeat that continued their record of not winning their opening two league games of a season for 37 years.
Victory also gave the Bluebirds revenge for their defeat at Pride Park as last season came to a close.
