Telford edged out Nuneaton to set up a Blue Square Bet North play-off final with Guiseley, winning their semi-final second leg 2-1 for a 3-2 aggregate win.
Jon Adams' perfectly placed header put Telford ahead and Sean Newton made it 2-0 with a penalty right on half-time.
Nuneaton fought hard after the break, Danny Spencer reducing the deficit.
"I'm immensely proud of all my players. The support from the fans was unbelievable," Telford manager Andy Sinton told BBC Radio Shropshire.
Guiseley reached the final after they beat Boston 3-2 in a penalty shoot-out in the other semi-final.
Boston won the second leg 3-2 after extra-time to make it 3-3 on aggregate.
