Zamalek needed a penalty shoot-out to reach the third round of the Champions League, edging past Africa Sports of the Ivory Coast in Abidjan on Sunday.
The five-time winners from Egypt lost the second leg tie 2-0, forcing the match to penalties.
But they managed to stave off a surprise early elimination with a 5-4 win in the subsequent shootout.
Jean-Michel Sery netted two penalties during the match to give Africa Sports the home win.
Sery's goals evened up the aggregate score after Zamalek had won the first leg in Cairo 2-0 a fortnight ago.
But team mate Peter Tayoro missed a vital kick in the shoot-out to hand Zamalek a third-round place.
Angola's InterClube also narrowly avoided an embarrassing exit as midfielder Minguito conjured up a last-minute win to give them a 1-0 win over minnows Sporting Praia from the Cape Verde Islands.
The second leg win in Luanda allowed the Angolan champions to progress on the away goals rule and end the fairytale run of Sporting, who had knocked Morocco's FAR Rabat in the first round.
InterClube had earlier squandered a penalty while Sporting had a second-half strike ruled offside.
Luyeye Mvete handed TP Mazembe Englebert, three-time winners of the Champions League, a precious 1-0 away win over FC105 Libreville of Gabon to see the Congolese side go through 2-1 on aggregate.
In Dar-es-Salaam, Enyimba of Nigeria beat their Tanzania hosts Simba 3-1 for a comfortable 7-1 aggregate triumph, including another goal for Stephen Worgu, who has now netted eight in four Champions League matches this year.
Enyimba chairman Felix Anyansi Agwu says the club is on the rise after missing the group stages of the two previous competitions.
"We missed out for two years but now we are back. We have shown a strong signal that something good is coming from our side this year," Agwu told BBC Sport.
"Simba created negative impression among its fans about our first leg victory that we beat them unfairly so we needed to prove a point to them."
Earlier in the day, Mamelodi Sundowns also reached the third round after their 1-0 away win over Curepipe Starlight of Mauritius.
Former Zimbabwe international striker Peter Ndlovu scored on 32 minutes to secure the victory for the South Africans at the New George V Stadium.
The 36-year-old finished with a firm right-footed shot that gave Starlight keeper Desire Francois Ammomoothoo no chance to seal a 4-0 aggregate win.
Sundowns progress to the third round to face Sudanese side Al Hilal, who saw off the challenge of Zesco United on Saturday.
The Zambians managed a 1-1 draw in Ndola but Al Hilal qualify 3-1 on aggregate after winning 2-0 in the first leg in Khartoum.
Holders Etoile du Sahel also progressed to the third round despite losing 3-0 away to AS Douanes of Senegal on Saturday.
The Tunisians advanced on a 5-3 aggregate thanks to their emphatic first leg win.
Two goals from striker Fadio Diao helped Douanes to the victory but their course was aided by the early expulsion of Etoile's Ghanaian striker Sadat Bukari.
Bukari was sent off on five minutes for elbowing an opponent and his side held out till late in the game.
Etoile's challenge crumbled in the 84th minute when Diao opened the scoring and Babacar Mbengue doubled their lead three minutes later.
Diao's strike on the stroke of full time was brave but not enough to guarantee them a place in the third round.
Elsewhere on Friday, former African champions Club Africain also went through to the third round thanks to their 4-0 win over ASKO Kara.
The Tunisians needed a big win in the second leg to advance after losing 2-0 to the Togolese in the first leg.
Club Africain came rushing back with the victory on home soil but it took them nearly half an hour to break the deadlock.
Lassaad Ouertani struck home after taking advantage of a slip-up by ASKO Kara defenders.
Seven minutes later the 1991 winners were level, after Oussama Sellami converted a penalty.
Six minutes after the break, Youssuf Mouihbi put Club Africain ahead in the tie with their third of the night, and their progress was assured with 20 minutes to go when Wissem Ben Yahia scored their fourth and final goal.
Last season's semi-finalists Al Ittihad of Libya are also through to the third round despite losing 2-1 to Primeiro Agosto in Angola.
But they won the first leg at home 1-0 and go through on the away goals rule.
