Johnstone's Paint Trophy holders Swansea were held 1-1 at League Two side Walsall, but went through on a penalty shoot-out at Bescot Stadium. Tom Williams hit the winning penalty after Kris Taylor and James Constable missed Walsall's first two spot-kicks.
Owain Tudur Jones had put the Swans ahead in the 90th minute with a driven shot from the edge of the area.
The Saddlers' Darren Wrack hit straight back from close range in his first game in 13 months, but it proved in vain.
Defences had been on top for most of the game, although Hector Sam rattled the bar early in the second half.
Swansea - who transfer-listed top-scorer Leon Knight on Monday - started with Lee Trundle as a lone striker, but brought 17-year-old Chris Jones on to partner him after the break.
