Reading will play Blackburn Rovers on Saturday wearing a one-off kit designed by 14-year-old fan Ryan Duval.
Duval drew the kit for a school project and Royals captain Jem Karacan suggested it be turned into reality after he visited the school.
The Tilehurst schoolboy has a condition called Duchenne muscular dystrophy, which restricts his movement and requires him to use a wheelchair.
Striker Simon Cox said: "It's brilliant to see this come to fruition."
Cox, who also visited the school to see the design in progess, added: "To meet Ryan, to see where it all began and meet the others along the way has been really nice. Saturday will be a touching occasion."
