General
If you are unable to log in using Single Sign-On (SSO), check that you are logged in with your BBC credentials and that your account has been set up for access.
If the issue continues, clear your browser cache or try an alternative browser.

- Requesting an update to a workflow or approval step.
- Reporting a form or dashboard error (for example, not loading or showing incorrect data).
- Adding or updating a user role or email domain.
- suggestions for new fields, values, or filters in a form or report.
Your request will then go to the BBC Product Support Team for review and action.
Auto-save is supported only in specific areas.
Always use Save before navigating away to avoid losing progress.
Activity Stream is mainly to record Status transition and notes regarding that; all other changes to the record are kept under 'Versions' (top right hand corner of the page)
Selecting Subtree in your filters lets you view the entire hierarchy — the chosen category and all categories beneath it.
It’s the quickest way to make sure you’re seeing the full picture.
Division x is in subtree of x (select from dropdown list)
Risk Assessment
This is as per design.
However, approvers should review the completeness before approving.
No. Items cannot be deleted once created. This is to maintain a full audit trail.
If you are the creator, you may be able to cancel the item instead of deleting it.
Only the Created By user can cancel an RA while it is in Draft.
Approvers and editors cannot cancel items created by someone else.
Use the Notes option (speech bubble icon) in the top-right corner to record additional information.
This happens when the Approver / Reviewer selects Further Work Required or requests updates. Your RA is returned to Draft so you can make changes before resubmitting.
Select the "..." icon - More Options (top right of screen) and select Copy Risk Assessment.
A pop-up window will open where you can select what options you want copied over and you can check the details contained within the copied RA and modify as required.
Three areas within each hazard are new and need to be entered:
- What might happen
- Risk Rating before Controls (5x5 matrix)
- Risk Rating after Controls (5x5 matrix)
'What might happen' needs to have details added to open-up access to the Risk Rating 5x5 matrix fields
Select the field with No Value, fill-in the missing details and save. The Risk Rating before Controls will then be available for scoring. Following a check to ensure the existing controls are still relevant/current, the Risk Rating after Controls can then be scored.
Controls migrated from the old system may be cut off if they exceed the character limit.
Create new controls using shorter, single-line text to avoid truncation.
Character limit in new system set to 500.
No. Controls on the form cannot be edited.
If you need to change it, delete it, and add it again.
(Controls in the Control Library can be edited depending on permissions.)
The system automatically reorders controls in the downloaded version.
This is expected behaviour and part of the design.
The table view shows a limited set of fields.
To see all RA information, select List View, which displays; Approver, Reviewer, Due Date, and other details.
It will not appear unless the RA is submitted. Drafts remain in your Draft section.
Incidents
Legacy incidents remain in their legacy categories if there was no matching category in the new platform; so they are not automatically migrated into the new category structure. This is expected behavior to preserve historical accuracy and prevent data misalignment.
Yes. You can edit legacy incidents category as long as they are still Open. If you need to update a cateogry for a Closed Incident please see - How do I raise a system issue or feature request? (General)
If someone records an injured person and is BBC staff. If the person has line manager, then a notification will be sent out. Injured person record and the incident records get updated which is when the notifications get triggered.
Trade Union users can view incidents for their area, but injured person details are restricted for data protection reasons and is visible to the Trade Union H&S Representative named on the injured persons record (individual injured person permission required)
A notification is sent to the person who raised the incident. The user who closes the incident will not receive a notification.
The table view shows a simplified set of fields.
Non-BBC locations may not appear in the default column. Use List View or clear filters to see the full details.
Adjust the filters or switch to List View. Some filters may limit what is displayed.

Use the filter panel at the top of All Incidents.
Set Incident Category → is equal to → Security or add Title → contains → security to narrow results.
Mobile App
This may happen if the app is not updated, if you have a weak internet connection, or if you are using an older device. Try updating the app, restarting it, or reconnecting to a stronger network. If issues continue, reinstall the app or contact the Safety Team for support.
Note that for the initial download of the app, it can take up to 5 minutes to download all necessary data so you might see the spinning wheel for a bit while that takes place.
The mobile app is designed as a field reporting tool, so it only displays records that are directly associated with you.
This includes:
• Records assigned to you
• Records where you are the owner
• Records where you are named on the record
The mobile app will not show all records you can view in the web app. The web app provides full visibility based on your permissions. It functions as a management tool, so it shows all items your role allows you to access.
The mobile app uses a simplified layout to support quick completion on smaller screens. All required fields are included, but the order and layout may differ slightly.
Yes. You can upload images and attachments directly from your device when completing an RA, incident, or assurance on the app
Draft items on the device where they were created. Once you submit the item, it will appear on both mobile and desktop
Submitted items sync automatically, but this may take a few minutes depending on your internet connection. Refresh the desktop browser if it doesn’t appear straight away.
Surveys (Assurance / Inspection must be created or owned on the web before they will appear in the mobile app
Survey drafts do not sync between platforms:
• Draft created on mobile → appears only on mobile
• Draft created on web → appears only on the web
