Are we discouraging children from learning to read?

A recent survey reported that only one child in five is able to identify a favourite book. And 20% of 11-year-olds are not reaching the required standard of reading. 

Is our formal approach to literacy discouraging children from reading? Is this a natural consequence of our multi-media society? When is the right time to introduce reading? Did the way you were taught to read influence your later life?
