Danielle Robinson was 21 when she took her own life in the Heddfan Psychiatric Unit at Wrexham Maelor Hospital in Wales.

In May, an inquest into her death pointed to failures in the unit's patient observation policies which have since been reviewed. But even in the years leading up to her death, her parents had serious concerns about the lack of help available to her.
