"We all need to learn the lessons of what took place in 1995"
General Sir Nick Carter, Chief of the General Staff of the British Army told presenter Sarah Montague: " We have learnt a lot of lessons from it, and the implication of that is things were not right at the time".
He also said: "We recognised that we did not have enough instructors in ratio to those in training, we did not have a philosophy that we now do about training in rather then selecting out and indeed we did not have enough transparency"
