Hoax nurse Jacintha Saldanha 'not disciplined by bosses'
Bosses decided not to sanction a nurse who put through a hoax phone call at a hospital where the Duchess of Cambridge was a patient, an inquest has heard.
Hospital chief John Lofthouse said Jacintha Saldanha had made a "mistake" but she and another nurse were victims.
Mrs Saldanha, 46, was found dead on 7 December 2012, days after taking the call from two Australian radio DJs.
