Tony Blair's former advisor has said the so-called "dodgy dossier" on Iraq contained a "simple but serious mistake" in including information from an academic as if it was intelligence.
Alastair Campbell told the Iraq Inquiry he would defend the "integrity" of the earlier September dossier "to the end of my days", but accepted the February dossier had been lacking "quality control".
