Balls: Baby P case 'unforgivable'

A second serious case review into the death of Baby P has concluded it "could and should have been prevented".

The Children's Secretary, Ed Balls, who commissioned the review after concerns over the findings of the first report said he was determined to make sure the case would never be repeated.
