An inquiry into child abuse at Catholic institutions in Ireland has found church leaders knew that sexual abuse was "endemic" in boys' institutions.
The inquiry, which took nine years to complete, concluded that priests and nuns terrorised thousands of children for decades while government inspectors failed to stop the beatings and rapes.
But John Kelly, a spokesman for the victims, said the report did not go far enough.
