Danny Grewcock has been ruled out of England's next two Pool C games with a broken toe.
And scrum-half Matt Dawson is only rated 50-50 for Saturday's vital head-to-head with South Africa.
But on a positive note, both Andy Gomarsall, who had been struggling with a leg injury, and wing Iain Balshaw will be fit for selection.

Grewcock picked up his injury during the warm-up for Sunday's game with Georgia.
Starting the game from the bench, he damaged his toe following a clash with wing Ben Cohen in the warm-up. Scans later revealed the toe was broken.
England boss Clive Woodward said he was confident he had enough cover in the second row in Martin Johnson, Ben Kay and Martin Corry for the games against South Africa and Samoa.
And he said in an emergency Grewcock could still be called upon to play.
Dawson, deemed the most serious injury concern, is battling to overcome a knee complaint.
But Woodward insisted he would be fully fit in time to meet Samoa in their third pool match on 26 October and remained upbeat even with the injury setbacks.
"It's been a good news day," said the England boss. "No one is going home and the only one who might not be playing who would have been is Dawson.
"It's been no different to after any Test - it's just more dramatic because you have to fly someone across the world."
And Johnson echoed those sentiments, saying: "Injuries are part of it, every team will have them, especially if you haven't played for four or five weeks.
"But we have got a deep squad and we're used to training for a Test week with some guys missing sessions."
Another of England's walking wounded, centre Mike Tindall, will return to training on Thursday. Tindall suffered minor bruising on his knee during the Georgia game.
Reserve number nine Kyran Bracken's back injury remains a concern but England's medical staff are hopeful the damage will clear up in time for him to return to training on Wednesday.
In the meantime, Bath scrum-half Martyn Wood arrived in Perth on Tuesday.
However, Woodward insisted Bracken would be given until Thursday night to prove his fitness. Should he not, his place in the squad will be taken by Wood.
