Lions captain Brian O'Driscoll, flanker Richard Hill and centre Tom Shanklin have all been ruled out of the Lions tour of New Zealand. O'Driscoll dislocated his right shoulder in the first minute of the first Test on Saturday.
Hill damaged his knee in the same game and was substituted after just 19 minutes to be replaced by Ryan Jones.
And Shanklin, who was not included in the first Test squad, is out because of inflammation in a knee injury.
O'Driscoll was injured after an incident involving New Zealand hooker Keven Mealamu and skipper Tana Umaga right in front of the touch judge.
Head coach Sir Clive Woodward has requested video tapes offering different angles of the incident and hinted he may take the matter further.
"Brian is not good. It is a very bad dislocation, and it took 25 minutes to get his shoulder back in," said Woodward.
"I don't think a lot of the team saw it - it was off the ball. The pictures won't lie so we have just got to see what happened.
"It has been a bad day. Richard Hill has got a serious knee problem, and Tom Shanklin is out of the trip on medical advice."
Hill has damaged his medial meniscus ligament, but the Lions medical staff said in a statement on Sunday that a scan was "suggestive" of damage to his anterior cruciate ligament, a much more serious injury.
The statement said a "full assessment" would be made "over the coming days".
No decision has yet been made on whether any replacements will be called up and the new captain will be announced when the team for the second Test is named.
