The other nominees are Bristol scrum-half Shaun Perry, Northampton fly-half Carlos Spencer, Wasps wing Tom Voyce and Sale flanker Jason White.
Prolific Leicester wing Tom Varndell is one of five nominees for the Land Rover Discovery of the Season award.
The others are Leeds wing Tom Biggs, Northampton centre Jon Clarke, Sale flanker Magnus Lund and Newcastle's revitalised centre Mathew Tait.
The three nominees for O2 Director of Rugby of the Season prize are Richard Hill, who kept Bristol up on their return to the Premiership, Philippe Saint-Andre, whose Sale side secured top spot in the regular season, and Brian Smith, who has guided London Irish to the play-offs.
The winners will be announced at a ceremony at Lord's Cricket Ground on Wednesday, 24 May.
Catt and Smith already have one award each in the bag, having been named Player and Director of Rugby of the Month respectively for April.
