Italy scrum-half Pablo Canavosio has been ruled out of Saturday's Six Nations match against England at Twickenham because of fatigue.
Fabio Semenzato, brought into the squad after usual number nine Edoardo Gori injured a shoulder in last week's loss to Ireland, will now make his debut.
Team boss Luigi Troiani said Canavosio told them he "didn't feel up to being possibly used for 80 minutes".
Canavosio, capped 23 times, will be on the bench if needed.
In other changes made by coach Nick Mallett, injured flanker Josh Sole is replaced by Valerio Bernabo, fly-half Kris Burton drops to the bench with Luciano Orquera replacing him and lock Carlo Del Fava comes in for Santiago Dellape.
Extended highlights - Italy 11-13 Ireland
Italy were beaten 13-11 by Ireland in their opening Six Nations game, while England began their campaign with a 26-19 victory over Wales at the Millennium Stadium.
Italy have never beaten England in 16 previous encounters including 11 in the Six Nations, although last season's game in Rome was close, with the visitors edging it 17-12.
For England to suffer a shock defeat, Italian prop Martin Castrogiovanni believes his team-mates have to take the attack to the hosts.
"We cannot think we can defend for 80 minutes because if we do that then we will crash after 60 minutes," said the 29-year-old who joined Leicester from Italian side Calvisano in 2006.
"We have to go out on the pitch without thinking too much about what England will do. We have to respect them but think of doing our job and try to go out and win.
"From a physical level, we face a huge battle. We have to do better at using our three-quarters and vary our play a little bit. But as always we will need heart and will."
