Colin Calderwood's Forest could have sealed the win in normal time but John Danby saved Grant Holt's spot-kick after he brought down the striker.
Holt had a header tipped over and Julien Bennett missed with a volley.
Chester's Simon Yeo and Kevin Roberts missed their penalties leaving Emile Sinclair to score the decisive kick.
