Defending champions France advanced flawlessly to the finals with a 3-0 victory over Israel in Paris.
France, who have won all eight of their Group One games, opened the scoring through Arsenal striker Thierry Henry.
David Trezeguet doubled the tally with a powerful volley in the 25th minute and Jean-Alain Boumsong grabbed the third.
Slovenia, who were already assured of a play-off spot, settled for a 2-2 draw with Cyprus.
Group 2
Winners: Denmark
Runners-up: Norway
Former European Championship winners Denmark booked their place in the finals after securing a 1-1 draw against Bosnia-Herzegovina in Sarajevo.
Forward Martin Joergensen gave Denmark a 12th minute lead after he picked up Thomas Gravesen's pass.
Bosnia equalised six minutes before the break when top-scorer Elvir Bolic headed home his 17th international goal from a Hasan Salihamidzic cross.
Gravesen was sent off in the dying minutes for a second bookable offence.
Norway claimed second place after beating Luxembourg 1-0 in Oslo thanks to a Tore Andre Flo goal.
Group 3
Winners: Czech Republic
Runners-up: Holland
The Czech Republic, who had already qualified for the finals before Saturday, snatched a 3-2 win against 10-man Austria with a stoppage-time goal from giant striker Jan Koller.
Holland head for the play-offs but will be in confident mood after demolishing Moldova 5-0 in Eindhoven.
Group 4
Winners: Sweden
Runners-up: Latvia
Latvia surprisingly grabbed one of the play-off spots after beating Group Four winners Sweden 1-0 in Stockholm to edge out Poland, who beat Hungary 2-1 in Budapest.
Latvia survived the dismissal of defender Dzintars Zirnis on 73 minutes after Maris Verpakovskis had scored the vital goal midway through the first half.
Group 5
Winners: Germany
Runners-up: Scotland
Germany never looked back after Bayern Munich midfielder Mikael Ballack gave Rudi Voeller's side a ninth minute lead.
Fredi Bobic and Kevin Kuranyi wrapped up the points for Germany with second-half goals.
Scotland's narrow victory over Lithuania ensured Berti Vogts' side snatched the play-off spot ahead of Iceland, who had started the day in second place.
Manchester United youngster Darren Fletcher gave Scotland a 1-0 win with a well-executed volley.
Group 6
Winners: Greece
Runners-up: Spain
Greece sealed their place at Euro 2004 with a 1-0 home win over Northern Ireland.
A 69th-minute Vassilis Tsartas penalty undid the visitors' stubborn resistance after George McCartney was sent off for bringing down the charging Zisis Vryzas inside the area.
Spain took second place after beating Armenia 4-0 in Yerevan.
Group 7
Winners: England
Runners-up: Turkey
England held on to a valuable point in Istanbul to go through as group winners after an impressive unbeaten run in qualification.
Sven-Goran Eriksson's side put in a spirited display against Turkey and despite a dreadful penalty miss by David Beckham, the goalless draw was enough. Turkey will hope to find a route to Portugal through the play-offs after finishing just one point behind England in the group.
Group 8
Winners: Bulgaria
Runners-up: Croatia
Croatia grabbed a place in the play-offs after a tight 1-0 victory over Bulgaria, who had already guaranteed their passage to the finals.
Ivica Olic came off to bench to seal maximum points for Croatia with an easy tap-in after 48 minutes.
Belgium finished with the same number of points as Croatia after beating Estonia 2-0 but missed out on goal difference.
Group 9
Winners: Italy
Runners-up: Wales
Italy strolled into the finals with an emphatic 4-0 win over Azerbaijan leaving Wales needing to qualify through the play-offs.
Fillippo Inzaghi, whose hat-trick broke Welsh hearts last month, scored a brace with Christian Vieri and substitute Marco di Vaio also on target.
Mark Hughes' side were stunned 3-2 by Serbia & Montenegro at Cardiff's Millennium Stadium.
Celtic striker John Hartson equalised from the penalty spot after Zvomimir Vukic had put the visitors ahead in the fifth minutes. But two late strikes from Savo Milosevic and Danijel Ljuboja rocked Wales, with Rob Earnshaw's injury-time header little consolation.

Winners: Switzerland
Runners-up: Russia
Switzerland dashed the Republic of Ireland's qualification hopes with a comfortable 2-0 win over Brian Kerr's side in Basle to reach the finals of a major tournament for the first time since Euro '96. Hakan Yakin capitalised on a mistake by John O'Shea to give the Swiss a sixth minute lead and Alexander Frei added a second on 60 minutes.
Russia came from behind to beat bottom-placed Georgia 3-1 and secure the play-off spot thanks to goals from Dmitry Bulykin, Yegor Titov and Dmitry Sychev.
