Tranmere keeper Danny Coyne pulled off a fine late penalty save to earn Rovers a place in the FA Cup second round. Jack Lester fired Chesterfield into a lead with his 15th goal of the season, but Chris Greenacre levelled from the spot after Chris Shuker was fouled.
Steve Fletcher had a header saved before the Tranmere went in front when Anthony Kay volleyed in from 15 yards.
The tie ended dramatically when Coyne saved Lester's late penalty before Tranmere had Craig Curran sent off.
