A late strike from Tom Pope broke Chesterfield's resistance as Rotherham claimed the derby spoils.
With moments remaining, Pope turned on the edge of the area to fire a low shot beyond Spireites keeper Tommy Lee.
Rotherham had earlier spurned a chance to go ahead when Adam Le Fondre shot at Lee after an Aaron Downes slip.
Jack Lester and Downes, whose dipping volley drew a good save from Rotherham keeper Andy Warrington, both threatened for Chesterfield, but to no avail.
Chesterfield manager John Sheridan told BBC Radio Sheffield:
"I could see 0-0 written over that match. It was a typical derby game and we didn't deserve to lose it.
"It's more of a case that we switched off. From the first minute to the last you have to stay focussed and we didn't do that.
"There wasn't anything in the game. I'm really disappointed to lose the game and I did think there was a lot of effort from our players."
Rotherham manager Ronnie Moore told BBC Radio Sheffield:
"The game was a typical derby game and both teams had chances but I think we had the better chances.
"It was a good win and it's nice when it happens here.
"It was a great finish from Pope and let's hope he'll get more goals."
