Adam Stansfield struck in the 88th minute to earn 10-man Exeter a draw and upset Millwall's promotion charge.
The Lions grabbed the opener against the run of play when David Martin picked himself up after being felled by Bertie Cozic and scored from the spot.
And the visitors looked to be well on course for the win when Exeter's Steve Tully saw red for a second booking.
But they wasted a number of chances to wrap it up and Stansfield punished them when his cross crept in late on.
