The visitors required seven runs from the final over with four wickets in hand, but Hall bowled Warren Hegg and Peter Martin before John Wood was run out.
Mal Loye, who batted through the innings to end on 116 not out, was left needing an impossible seven runs from the final delivery.
The result seemed to be in the bag for Worcestershire when Hall and Matt Mason bowled brilliantly in tandem, removing Lancashire's highly-regarded trio of Stuart Law, Carl Hooper and Andrew Flintoff for a combined 24 runs.
But brilliant innings by Chris Schofield (32) and Glen Chapple (44), keeping the tiring Loye good company, swung the pendulum back Lancashire's way.
With 15 needed off the last two overs, Kabir Ali bowled Chapple - and that ultimately proved to be the turning point.

Lancashire's first error of the day was to choose to field first on a dry batting track which was at its best in the early part of the day.
Graeme Hick accepted the invitation to hit a brilliant 97 - and only some fine bowling at the death from James Anderson and Flintoff kept the score down to manageable proportions.
Former England batsman Hick, coming off a long injury lay-off, pulled and drove with tremendous effect.
Anurag Singh added 155 with the veteran, contributing a handy 63 runs of his own.
But it was Hall who provided the necessary acceleration in the latter part of the innings, improvising to slam five boundaries late on after Hick and Singh were out.
Worcestershire now meet Gloucestershire, who downed Derbyshire by a single wicket on Thursday, in the 30 August final at Lord's.
