Worcester wing Thinus Delport scored two tries to give the basement club a priceless victory over Bath.
The home side had the perfect start when Delport dived over on nine minutes although Chris Malone replied after he charged down Shane Drahm's clearance.
Drahm added three penalties to Olly Barkley's one and Worcester took control when Delport scored his second.
Bath, who lost Steve Borthwick to a rib injury, did cross through Shaun Berne but it was too little, too late.
Worcester, whose last home league success came against Newcastle more than 10 months ago, have now closed the gap on 11th-placed Northampton to four points.
And they had the perfect start when Delport dived over following a well-worked move.
The home side continued to threaten but Bath equalised through Malone after Drahm attempted to clear from a Craig Gillies mistake.
Worcester looked the more threatening and Shane Drahm put them 10-7 ahead just before half-time.
Borthwick was forced off on 45 minutes but Barkley equalised for Bath soon afterwards.
Drahm added another two kicks and Delport made sure of the points when Matt Powell sent him over seven minutes from time.
Bath, who have not won away in the league for over 12 months, did claim a bonus point late on through substitute Berne.
Worcester director of rugby John Brain:
"Bath have a strong set-piece, and I thought the key to the game was that we managed to undermine them in those areas. "We dismantled their line-out, which not many teams manage to do.
"We gave ourselves a good platform up front, although I was disappointed we took our foot off the pedal after scoring our second try."
Bath head coach Steve Meehan:
"I am bitterly disappointed. Our set-piece didn't function, and we gave away too many turnovers. You cannot expect to win games if that happens. "Not winning away from home has started to annoy the hell out of us. It is inexplicable, really.
"We talked all week about attacking Worcester's defensive pattern, and we did that for 10 of the 80 minutes, then we played into their hands."
