More than a quarter of NHS trusts in England may not be offering safe, quality care to patients, according to the Care Quality Commission.

Each trust has been put into one of six bands. Bands one and two are the ones classed as high risk, while five and six are the lowest risk.
Not all the indicators are applicable to every trust - so trusts have only been assessed against the one relevant to them and a proportionate risk score has been calculated.
Where serious concerns have been expressed about a trust already, such as being placed in special measures, the organisation is automatically placed in band one regardless of the risk score.
The trusts marked with an * are those that have been elevated for this reason.
