Charity donation
They are designed for all members of the community in clay country including those who work for Imerys.
People are being urged to complete the forms and send them back so the company can find out what they think of the way Imerys land is used and how they are affected by operations.
It also includes a section on the future of the Trelawney Tigers Speedway track at Lower Longstones and the issue of tipping.
Recently the clay giant has been given permission for another waste tip at Scarcewater in St Stephen.
It says if all questionnaires are returned a total of 13,000 will be donated to charity.
