Metropolitan Police Acting Commissioner Sir Paul Stephenson said police made a 'most terrible mistake' in the shooting of Jean Charles de Menezes.
He said it was his job to make sure that lessons had been learned.
It came after the jury at the inquest returned an open verdict.
