The World Health Organization says it is no longer possible to contain the outbreak of swine flu to Mexico, where it is believed to have claimed around 150 lives.
Officials have raised the alert level from three to four, two steps short of declaring a full pandemic.
Gregory Hartl from the WHO in Geneva said countries must be in a state of readiness.
