The BBC must be "firmer on excesses" after the prank call row, but must still take risks, the BBC Trust chairman Sir Michael Lyons has said.

Russell Brand and the controller of Radio 2, Lesley Douglas, have both resigned in the wake of the row surrounding calls made to the actor Andrew Sachs.

Jonathan Ross has been suspended from all BBC programmes for 12 weeks without pay.

Nick Higham reports.
