Lord Cullen's inquiry into the Paddington rail crash has published a damning report into how the disaster happened. Railtrack, in particular, comes in for heavy criticism. No part of the rail industry escapes censure.

Firefighter Sally Cox says none of her training could have prepared her for the carnage she witnessed in the aftermath of the Ladbroke Grove rail disaster. Mrs Cox, whose picture was plastered across many newspapers in the crash aftermath was involved in the arduous task of locating and freeing bodies from one of the trains.

An interim report has blamed Britain's worst rail disaster for decades on failures within the railway system. The Health and Safety Executive said the immediate cause of the crash appeared to be a commuter train jumping a red light, but it said the root causes were likely to be complex.

The trains collided at the height of the morning rush hour. A two-carriage service run by Thames Trains from Paddington to Bedwyn, Wiltshire, collided with an incoming Great Western 125 express train from Cheltenham. Carriages came off the track and one train burst into flames.
